If your child will be absent all day, arriving late, or leaving early, please report the changes in attendance to the school office, not the teacher. The office will make sure your child's teacher(s) are notified, and pass along any requests or information you wish them to have. If you notify the teacher directly, he or she may not realize the office has been notified, resulting in an unexcused absence and message to you. You may use any of options below to communicate your child's attendance. If you prefer, you may also call the offices: Lower Campus 616-447-2997 Upper Campus 616-363-7725.

Student Health and Information Form: Please complete one form for each student. These forms are used in our classroom emergency folders; therefore, it is important that we have one for every child.

Child Care and Preschool MI Child Info Card - These go in our classroom emergency folders, so it is important that we have one for every child.
Health Appraisal - Must be signed by a physican by the start of the school year.

The school names are "ALL SAINTS ACADEMY LOWER CAMPUS" and "ALL SAINTS ACADEMY UPPER CAMPUS". There is no password.

Unfortunately school staff are legally unable to provide medications of any kind for a student without documented parent permission, including dosage information. Completing the applicable form(s) below makes it possible for your child to take meds while at school.
