If you manually fill out the PSAS form you must mailed it along with photocopies of the required documents and a check for $30.00 made out to PSAS. Please use the address on the application.
If you choose to complete your application online, you have the choice to send the required documentation several ways. You can upload them, fax them or mail them to PSAS. Instruction for each are listed
on the link above. Please note that the address for mailing this documentation is different than the mailing address for those who manually fill out the form. There is still a registration fee if you fill out the application online, but you may either send in a $30.00 check to either school office made out to All Saints Academy for the PSAS fee or $30.00 will automatically be added to your smart tuition account.
If you need a spanish version of the PSAS form, please contact the school office. It is only available as a printed form at this time.
